Patient Forms

If you are a Prompt-Pay (time of service) patient or have Insurance Coverage, please visit our Forms page.  You will need to download the intake forms, informed consent/arbitration agreement and financial agreement. If you are able to complete these, prior to your appointment, this will allow us to attend to you more quickly.  Thank you!!


AS of 2019, we will now be requiring a credit card to hold your appointment. If you cannot make your scheduled appointment, please give us at least a 24 hour notice, call and leave a message if you do not get a hold of anyone. A voice message within 24 hours of your appointment is acceptable. If you miss an appointment, forget an appointment or cancel with less than a 24 hour notice you will be charged $50. If the last minute cancellation is an emergency, the fee will be waived. We are reserving a specific time for you, please be mindful. We have an extensive waiting list, so if you cannot make your appointment, someone else may be able to take your place. If you are one of those on the waiting list, we will call you as soon as we receive any cancellations. Our first priority is our patients and their care, we know you value your appointments with us!

Please be mindful of wearing any strong smelling lotions, perfumes or cologne into the clinic.  Some of our patients have severe allergies to these products.

Please turn off cell phones before entering the treatment rooms.  We would like to keep a quiet space for everyone to be able to relax.

  • Initial visit $90 (~Approximately 1 hour and 30 minutes), this includes a consultation of all your concerns, evaluation and treatment. Treatment may consist of Acupuncture only or a combination of Acupuncture with Electric Stimulation, Cupping and /or Moxibustion, in which there may be additional charges. Herbs or supplements may also be included if needed.
  • Follow-up visits $80 (~Approximately 1 hour)
  • Herbal and Supplement Consultations (~30 minutes, separate from Acupuncture treatments), this includes a health history, tongue and pulse diagnosis. $60
  • Cupping or Gua Sha Treatments $50
  • Senior and Military receive a discounted rate. Initial Visit $80 and Follow up Visits $70    


Most insurance is accepted.  Please contact your insurance company to see if Acupuncture is a covered benefit on your policy.  If treatments are covered, there is usually a co-pay or co-insurance due at the time of visit. 

We can also verify your insurance, if you would like.  In order to verify insurance we will need:

  • Full name of primary subscriber
  • Name of patient, if different from primary subscriber
  • Date of birth of both subscribers
  • Name of Insurance co. and phone number for the Insurance co.
    • Aetna (very often, Aetna only covers for Acupuncture for anesthesia, this is not performed at our office)
