Please call for an appointment. Leave a brief message if you get the voice mail. You can also email us from the appointment page.
If you are a Cash patient or have Insurance Coverage, please visit our Forms page. You will need to download the intake forms, informed consent, arbitration agreement and financial agreement. If you are able to complete these, prior to your appointment, this will allow us to attend to you more quickly. Thank you.
If you cannot make your scheduled appointment, please give us at least a 24 hour notice, call and leave a message if you do not get a hold of anyone. If you miss an appointment, forget an appointment or cancel with less than a 24 hour notice you will be charged $50. If the last minute cancellation is an emergency, the fee will be waived. We are reserving a specific time for you, please be mindful. We have an extensive cancellation list, so if you cannot make your appointment, someone else may be able to take your place.
Please be mindful of wearing any strong smelling lotions, perfumes or cologne into the clinic. Some of our patients have severe allergies to these products.
Please turn off cell phones before entering the treatment rooms. We would like to keep a quiet space for everyone to be able to relax.
